VOLUNTEER EVALUATION FORM

In a effort to provide better service to those of you who have children involved in Little League, the District Administrator would like to hear from you.
This form is provided for your convenience. Please feel free to evaluate any of the volunteers on the District Staff or in your local Little League on this form. The form may be used for both positive and negative input. It is not designed to be a personal gripe form about any one individual.
You may submit this form anonymously. However, if you decide not to give the personal information requested, the District Administrator will not be able to provide a response (if desired) to your evaluation.
If you decide to give your name, it WILL NOT be released to any Local League Officials or District Staff members.
The only way any individual, other than the District Administrator, will find out who submitted this evaluation is if you tell them, or you authorize the District Administrator to tell them.
In any case your input is greatly appreciated. Thank you for your interest in improving the quality of the volunteer service in the Little League program, and especially in District 51.
