REGISTRATION FORM

TO PLAY LITTLE LEAGUE WITHIN

DISTRICT 51 FOR THE CURRENT SEASON

     District 51 in an effort to assist the eighteen (18) Leagues within its boundaries is providing this form to assist parents and players in the registration process. Please fill out the following form. It will be given to the proper League (the League whose boundaries you live in). The Player Agent for that League will then contact you with registration sign-up dates, as well as any other information you may need. The player must be at least 5 YEARS OF AGE AND NO OLDER THAN 18 YEARS OF AGE ON OR BEFORE JULY 31. Please note that District 51 encompasses ONLY the following towns/cities -- Acton, Palmdale, Lancaster, Lake Hughes/Elizabeth, Quartz Hill, Rosamond, Edwards AFB, Mojave, Boron, California City, Tehachapi, the Kern River Valley, Mono County, Bishop, and Owens Valley.
