COMPLAINT/COMPLIMENT FORM

In a effort to provide better service to those of you who have children involved in Little League, the District Administrator and his staff would like to hear from you. This form is provided for your convenience. Please feel free to list any complaints (or compliments) about your local Little League on this form. You may submit this form anonymously. However; if you decide not to give the personal information requested the District Administrator will not be able to provide a response to your problem or concern. If you do give your name, it will not be released to the Local League Officials. The only way they will find out who sent the form is if you tell them, or you authorize the District Administrator to tell them. In either case your input is greatly appreciated. Thank you for your interest in the betterment of the Little League program in District 51.
